I would like Fox and Hound to seek veterinary care for my pet if there are ANY concerns. I understand that I will incur additional charges and I agree to be responsible for any & all charges as deemed necessary by the Fox and Hound staff & the veterinarian providing the services.

I would like Fox and Hound to attempt to contact me prior to seeking medical care, and if I cannot be reached, I authorize my secondary contact to approve/disapprove care.

If I and/or my emergency contact can not be reached, I would like Fox and Hound to seek the treatment they feel is necessary.
